Referral Criteria

(for use by private clinicians)

  • A primary diagnosis of a Severe and Persistent Mental Illness (SPMI) (i.e. schizophrenia, schizo-affective, bi-polar, major depression with psychotic features, or psychosis NOS.)
   
  • Durham County resident
 
  • Age 18 or older
   
In addition to the initial criteria, we also screen for other criteria on a case-by-case basis.  These criteria do not rule out a client for admission, however we must consider them during the screening process.

  • All Axis II diagnoses, including all personality disorders and mild MR.  As this program is purposely understaffed to engage members in meaningful work activities throughout the entire day, these diagnoses demand more one-on-one time than the program is set up to provide.
    
  • Substance abuse:  We look for 30 days documented clean time.  However, we are mostly concerned with the type and frequency of use.  The client must be aware that coming to Threshold under the influence will jeopardize their membership.
    
  • Present behavioral problems and patterns of harmful behavior are considered.
    
  • Legal involvement:  This cannot be court-ordered service, as this is a voluntary program.
